Visa Notes

• Passport: Original Passport with validity of minimum six months and minimum two blank pages for visa stamp.

a) Photocopy of data and amendment pages of the applicant's passport.

b) Attach all your old passports (if any)

• Philippines Visa Application Form : One visa application form duly filled and signed by the applicant.

If applying from Mumbai:

Philippines Mum Visa Application Form : One visa application form printed back to back, duly filled and signed by the applicant as per passport.

If applying from Chennai:

Philippines Chennai Visa Application Form :One visa application form duly filled and signed by the applicant.

• Photo Specification: One recent passport size photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35mm x 45mm)

If applying from Mumbai:

One recent Passport size Photograph with white background affixed on the visa application form & signed across on front side.(Size: 50mm x 50mm)

Please note: Photograph should not be more than 3 months old,scanned/stapled and should not be used in any of the previous visas

• Covering-Letter: Covering Letter from the applicant on the company's letter head stating applicant's name, designation,passport number, purpose, duration of visit and number of entries applied for (single/multiple entry valid for 6 months or 1 yr).
  Please Note : Covering Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.

(If applying in Delhi)-The letter should be addressed to "The Visa Officer,Embassy of the Republic of Phillipines,New Delhi"

(If applying in Mumbai)-The letter should be addressed to "Honorary Consulate of Philippines,Mumbai"

• If sponsored an invitation letter is required stating the complete address and contact no of the invitee.

a) Copy of invitees Passport, Visa and Alien card.

b) Financial documents of invitee.

• Financials :

a) Company's and Personal ITR for the last 3 yrs.

b) Original Company and Personal Bank Statement for last six months mentioning the name, address and account no. with Bank's name, Bank's Telephone Number,Bank's stamp & signature of authorized signatory. Please note: If applying from Delhi - Minimum Balance in the Bank Statement should be Rs 80000.

c) Credit Card copy/Foreign Exchange endorsement/Traveler's Cheque

• Proof of Occupation

Employed

a) Certificate of the current employment from employer stating employer's full name address and telephone no., applicant's length of service and salary details and NOC Letter and salary slip for 6 months.

Self-employed

a) Photocopy of latest Income Tax Return

b) Copy of Personal Assessment Number (PAN) Card.

Student

a) Leave Sanction Letter, School ID Card and NOC from school mentioning the dates of trip

If minor, under 15 years of age traveling alone or to be accompanied by a person other than a parent or not joining a parent in the Philippines:

a) Affidavit of Support and Consent to Travel. The affidavit must be executed by either or both parents indicating traveling companion of minor and address in the Philippines, duly notarized by a local Notary Public, attested by the MEA and consular officer of the Philippine Embassy. This affidavit must be presented before immigration officials at the port of entry in the Philippines upon the minor's arrival in the Philippines.

If Spouse of a Philippine Citizen:

a)Name of Filipino spouse should be indicated in the visa applicant's passport Copy of Report of Marriage issued by the Philippine Embassy or Philippine National Statistics office

If Unmarried Minor Child of a Filipino Parent:

a) Copy of Birth Certificate and Copy of Parent's valid passports

• Proof of Accommodation : Original Hotel confirmation
• If applying from Delhi / Mumbai -
  Ticket : Confirmed Return Ticket with ticket number.
  If applying from Chennai-
  Airline Reservation : Ticket Itinerary
• Note:

a) If employed, a Certificate of the current employment from employer stating employer's full name address and telephone no., applicant's length of service and salary details is also required.

b) W.e.f 23rd April 2012, The Visa Fee for Same Day will be submitted in morning along with the application and even if Philippines Embassy is not able to issue Visa on the Same Day,the fee will not be refunded.

c) The Philippines Embassy in New Delhi insists on sighting Original Traveller's Cheque for a minimum of US$1500 along with original receipt. It is therefore advisable that you submit such Traveller's cheque upfront with the application.

d)Address Proof: If applying from Mumbai : Outstation applicants who submit rent agreement as proof of stay will also have to get the same verified from local Police Station and attach the Police Verification along with the agreement

• Indian nationals may be granted visa-free entry for a stay upto fourteen (14) days, provided they possess the following:

a) A valid U.S.A , Japanese, Australian, Canadian, Schengen, Singapore and U.K. visas or permanent residence permits

b) Passport with a validity for at least six (6) months, beyond the stay period

c) Return or onward ticket to the next country of destination

d) No derogatory record with the Bureau of Immigration (BI),the National Intelligence Coordinating Agency (NICA),and the International Police (Interpol).
